• ARRANGED ABSENCES

    PRE-AUTHORIZED ABSENCE FORM-If a student will be absent for one or more days of school due to a family, church, sport, or other elective activity, this form should be submitted at least 2 weeks before the absence occurs.  Please submit this form regardless of whether that absence will be excused or unexcused.   This form should also be completed if a student will be absent from a required performance (Veterans Celebration, Christmas Play, and Harmonic Festival)

    RE-CURRING ABSENCE FORM- If your student will be absent for recurring appointments (standing appointments, sports practices, ect.)

    Please fill out this volunteer application form if you are interested in being a volunteer at SCA.  All volunteers must have a form on file.  

  • Sports Participation Physical form

    SCA has a Cross Country Team.  To participate in this sport, your student must complete the following Sports Physical Form.
